There used to be a place on Yonge Street that did custom work. Fantastique Shoes, right near Isabella, but they've been gone for a few years now. I had beautiful custom boots from there that lasted me for *years*.
Some of the "western" boot places on Yonge might do a non-cowboy thing for you; they sometimes carry motorcycle boots and such, and do a lot of custom work, although that might just be custom embellishments.
Barring that, head down to the One of a Kind Craft Show in November at the Ex. There are at least two or three cobblers there with hand-made shoes; I'd bet any of them would do something custom for you, if you're willing to pay their somewhat higher prices.
